Document Verification & Evaluations

This weekend is the first date for document verification and player evaluations. All players must verify address and birthdate. Players league age 8 and above must attend one of the evaluation sessions. Both events will occur at Ponderosa Elementary on Saturday, January 22. If you can’t make it Saturday, there will be another opportunity next weekend.

 Document verification will be 9:30 am to 12 pm.

 The schedule for evaluations is as follows:

  • League Age 12 and returning Majors: Check-in 9 am, Start 9:30 am.
  • League Age 11 and 10: Check-in 10:30 am, Start 11:00 am.
  • League Age 8 and 9: Check-in 12 pm, Start 12:30 pm

Please head to the Ponderosa Snack Shack (just behind the backstop next to the play structure) to verify documents and/or check in for evaluations.

For new and returning players, bring

  • 1 proof of age (ORIGINAL BIRTH CERTIFICATE) and
  • 1 proof of residency from each of the below listed three groups:

1. Driver’s License (photo ID of parent(s) or guardian)

1. Homeowner or tenant records (i.e. lease or property tax bill)

1. Utility bills (i.e., - gas, electric, water/sewer, phone, mobile phone, heating, waste disposal)

2. Vehicle records (i.e., registration, lease, etc.)

2. Federal records (i.e., Federal Tax, Social Security, etc.)

2. Internet, cable, or satellite television records

3. Insurance documents (with residence address)

3. State records

3. Financial records (loan, credit, investments, etc.)

4. Employment records

4. Local (municipal) records

4. Medical records

5. School records (home address of player’s parent/guardian)

5. Support payment records

5. Voter’s Registration

 

6. Welfare/child care records
